Header: PFB the heading related issues(New UI Change)

Steps To Reproduce:

1.Requests needing approval: View approver comments link: Heading is blank
2.Doc Management- Draft history > View comments > Dupicate header
3.Groups: Groups Doc Pro heading should be in the breadcrumb and delete the header
4.Records menu:Doc num click->Records popup header
5.Change request:From Tag search,From ToC, From draft Actions(pending and draft req needing viewing): Heading shows twice
6.Pending requests/Pending record requests: Modify link->Modify Button->Change request header and breadcrumb
7.Site PDF Preferences heading and breadcrumb
8.New route details breadcrumb and heading
9.Doc mgmt: Doc review history->View comments link->Heading and breadcrumb
10.Doc Mgmt: Change history: No breadcrumb/header
11.Folder mgmt: Level PDF Preferences header
