Steps To Reproduce:
1) Login as auditor
2) Click on perform audit
3) Click on upload audit record
4) Fill the checklist
5) Submit the checklist
6) Click on Publish for approval
7) Note the revision number of the audit report
8) Publish multiple times and Observe the Revision number by clicking on Document number hyperlink
