Steps To Reproduce:

1.Login as boss user
2.Expand BOSS-Setup
3.Click on Risk-Templates or Data entry form template link
4.Select the existing form row and then click on edit button
5.Rename with duplicate form name
6.Click on save button

Description

Template -> The page should not allow to save with duplicate Form name, Description, No of rows, no of Columns.
Note: Same occurs in alignment Chart View - Right Click on Process -> Click on "Create Risk"
