Steps To Reproduce:
1.Login as suite admin(responsible person)
2.Click on Actions under Action management
3.Select the action name
4.Try to attach documents in the ECR form

Description
1.Responsible person is Not able to attach documents
2.On clicking view Risk the detail page opens up for resp person
3.Responsible person is not able to view the revision document on clicking the rev number
