Steps To Reproduce:

1)Login as project manager
2)Click on project menu
3)Open the active project
4)Click on Meetings icon
5)Schedule a meeting for the deliverable
6)Now open the meeting and add action plan
7)Observe the icon

Description

1)Change the concern icon and its status icon . Need same as that in project gantt page.
2)After initiating the concern clicking on the concern icon shows the same page(Initiate new concern).
3)Mouse hover on the concern icon section and observe the incorrect tooltip
4)Need a Open status for the action plan while adding .
