Steps To Reproduce:

1.Login as RFD module admin
2.Create PI and BOM
3.Add function/Requirement/Failure and Do flow down A to B
4.Now Add another function/Requirement/Failure for the same PI to PI
5.Now Expand the Arrow
6.The Function/Requirement/Failure are duplicated
