Steps To Reproduce:

1.Request new document draft
2.Go to actions
3.Select the doc from pending document draft
4.Click on Change request
5.Attach the file (attached in files)
6.The system keeps loading
7.Refresh
8.Login in as User B
9.Repeat the same
10.Attach the file
11. Pending Document displays twice with different requester
