Steps To Reproduce:

1)Login to ewqims application
2)Under Change Management
3)Click on Initiate Change Request menu
4)Select the change category
5)Click on Add Document
6)Select the document
7)Click on OK button(Green)
8)ECR Form is loaded but selected document not listed in the document section
