Steps To Reproduce:
1. Log into Audit pro as Scheduler.
2. Go to Setup tab.
3. Click on Process/Activities page.
4. Go to audit form column and Click "+" icon.
5. Add audit form and select form type as template and attch doc.
6. Now change form type as Online form and attach audit form.
7. Now change back to template and observe
