1. Log in as Champion.
2. Go to initiate problem solving.
3. Open any of the concern
4. Fill in CAR form.
5. Click on Nested approach button.
6. Click on all the fields and observe

Description

While creating nested approach, the root cause, corrective action field and category fields getting duplicated, because of this while creating child concern is getting duplicated.
