Steps To Reproduce:

1.Login as approver
2.Select wait for a doc action
3.Login as requester and check the approver comments in pending req/change history
4.Its displayed as NA
5.Assign author and attach the document
6.Now check the request details in pending requests
7.Comments shows NA and the status is inprocess
