Process / Activities - Some of the basic functionality is not working - Add using special character, Delete, Edit process

1) login as user
2) Click the Process/Activities
3) Click the add button

Using the special character data is not saved

1) login as user
2) Click the Process/Activities
3) Click the edit button

Edit contain is displayed corrupted

1) login as user
2) Click the Process/Activities
3) Click the delete button

Deleted functionality is not working
