Steps To Reproduce:
1. Login in RFD Module as Module Administrator
2. Click the Main Menu.
3. Click on the PI Tab select the PI
4. Click on the RFD Tab
5. Click on VOC
6. Click on Publish for Approval
7. Publish the document for selected level with auto approval
8. Click on Published document link
9. Observe the Revision Number field
10. Again publish the same document
11. Observe the Revision Number

Description
When the document is published undergoing revisions will be "0".If again publishing the same document, it should increment the revision as "1". But here it displays the second revision as "2".
