Steps To Reproduce:

1) Login to module as auditor(Multi site)

2)Click on perform audit

3)Click on upload audit record

4)Observe the upload audit result screen\

5) Page is displaying Blank

Description

Assign Audit form is not displaying in upload audit record page. displays blank for fill checklist.

The Same is not working for LPA assigned checklist is not displaying compliance in fill checklist displays blank though checklists are assigned.
