Corrective Actions -> Revision # is not working as expected

1. Login as auditee
2. submit corrective action response back to back by using different car approach
3. Observe the result in doc pro-documents page, actions page

Description

Corrective Actions -> Revision # is not working as expected.

The auditee submitting corrective action response after getting approval (Rev 0 to Rev1). But in doc pro-documents page, actions page its showing first revision # (Rev 0) always.
