Steps To Reproduce:

1.Login into EwQIMS Application
2.Select Consolidation
3.Click on the consolidated training list
4.Try to edit survey for second time
5.Observe the screen

Description

- While clicking on the consolidated training request for editing purpose the consolidation survey details are not get displayed.
