Steps To Reproduce:

1.Enter the site URL
2.Enter the user credentials
3.Click on module icon and select Boss module. 
4.Expand Boss_Setup tab from menu. 
5.Click on "Data entry form template" icon.
6.Add a form
7.Click View Designer
8.Observe the result
