Cant able to create new Concern in Problem Solver. After creating the concern is not showing in New concern grid also it is not listing in the Concern Initial Evaluation menu also

1. Log in as Admin.
2. Go to Actions tab.
3. Click on New Concern Menu.
4. Click on New.
5. Create and click on Submit.
6. Observe the result
