1.Log in EwQIMS 2.Navigate to problem solver module 3.Select Corp site 4.Click on Problem solver setup → Concern form 5.Add element select,multi select,radio,Checkbox 6.Enter form name click on save 7.Associate categories and sub categories 8.Create new concern 9.Again 9.again click on created conern form modify the elements 10.Add options for custom elements 11.Click on save 12.Observe result 13.Refer screenshot
