 2.Click on Suite Setup 
 3.Click on Doc Pro Setup 
 4.Click on Document Template 
 5."Default New document Request Form" should Show -- This is not showing only in Multi Site 

 1.Click on Setup 
 2.Click on Suite Setup 
 3.Click on Business Rules 
 4.Click on 'Set Fields for Doc type' 
 5.Select the Doc Type 
 8.The additional field are listed in the drop down
