Steps To Reproduce:

1. Log in as Resource.
2. Go to Action tab.
3. Move to survey tab.
4. Open the questionnaire.
5. Answer all questions.
6. Click on save.
7. Go to Doc Pro.
8. Expand the level.
9. Click on published level.
10. Click on Document and observe.
