Steps To Reproduce:

1. Login as supplier contact person
2. Click on "Self assessment report" link from menu
3. Click on Export icon
4. Save the file
5. Open the file and observe the result

Description

Supplier Report Export ->

1. CSV File type -> The field names are not same as report.

2. PDF File -> Unable to view the report fully (Not User friendly)

3. Word File -> Error message displayed while to try open or save
