Steps To Reproduce:
1. Login as auditor and then click on "Perform audit" link
2. Click on auditee / audit cycle hyperlink
3. Click on Add button under audit attachment link
4. Select attachment type as "Controlled" and then click on "Add files" button
5. Attach a document from local machine
6. Click on save button
7. Open the document by clicking on document name hyperlink
8. Observe the result
