1. Login as admin
2. Click on equipment link from menu
3. Right click on assembly line
4. Click on New Equipment
5. Create new equipment
6. Go to checklist tab
7. Create new checklist and click on + button to create new checkpoint
8. Observe the design

Description
1. The Checkpoint row is not user friendly
A. Checkpoint dropdown selection icon is out of row
B. Labels / Titles and a row(Dropdown, Radio button) is not in same order
