Perform audit -> Audit Forms -> Audit Form name is not displayed

Steps To Reproduce:

1. Login auditor
2. Click on perform audit link from menu
3. Click on any one audit cycle / auditee link
4. Go to audit forms tab
5. Observe the result

Description

Perform audit -> Audit Forms -> Audit Form name is not displayed. Instead of that "Link" is displayed.

Expeceted Result -> The form name should be displayed for any form type ( Reference / Template / Online form - Attachment or link)
