1.Login to EWqims
2.Go to problem solver module
3.Click on problem solver setup concern form
4.Click on new
5.Enter form name
6.Add select from available elements
7.Remove select from added elements
8.Add multiselect from available elements
9.Remove multiselect from added elements
10.Click on save
