Auditor Closeout attachments are not viewable after attach

Steps To Reproduce:

1)Login to module as auditor
2)click on corrective actions closeout
3)pick the respective audit
4)specify verified and validated as yes and no
5)attach the file click save
6)retrieve the page try to view the attachment

Auditor Closeout attachments are not viewable after attach

even after complete closeout the form is disappearing
