Steps To Reproduce:
1. Log in as Admin.
2. Go to JIC forms Tab.
3. Click on Pre wastage log form menu.
4. Select food item and submit.
5. Enter all data in all the fields.
6. Submit the form.
7. Click on pre wastage form again.
8. Select the food item and submit.
9. Observe the form
