Steps To Reproduce:

1)login to module as auditor
2)click on perform audit
3)click on upload audit record
4)observe the screen

Description

Uploaded audit record does not displayed the forms to perform audit.
when clicked on upload button in the empty window directly displays server error

Note: The problem is with the checklist and checkpoint the same will work when schedule is created using manual checklist and checkpoint
