Steps To Reproduce:

1)Login into EWQIMS application
2)Select APQP Module
3)Select the Project under Projects tab
4)Click on Document viewer
5)Select the deliverable and Click on attach icon
6)Attach the document
7)Now attach the revision document
8)Observe the result

This issue occurs for Output document,Additional document and Project Attachment
