In Survey page, While selecting rating in response tab, Questions in Question format tab needs check all check box, now it is missing.

Steps To Reproduce:

1. Log in as admin.
2. Go to survey tab.
3. Click on Survey menu.
4. Create rating.
5. Go to response tab, click on add.
6. Type response name and click on ">>" icon and observe the result.
