closedNC/OFI attachments when there is duplicate attachments for NC/OFI is getting updated and able to view but in audit details screen when clicked on view it is not listing.

Steps To Reproduce:
1)login to module as auditor
2)click on perform audit
3)raise NC/ofi
4)add attachment to nc/ofi same document 3 times and by 4th time add some other document
5)click on attach
6)click on done
7)click on view in nc/of screen in bottom of the screen able to see the documents
8)but in audit details screen when clicked on view not able to view the attached 4th document
