Steps To Reproduce:
1)login to module as auditor
2)click on perform audit
3)click on add audit attachments
4)attach the same document 3 to 4 times
5)the attached documents are getting over ride
6)go to doc pro and observe the assigned level

Description
1)Revisions are not updating after second revisions for add audit attachments in perform audit page
2)in upload audit record able to get duplicate document in doc pro after attaching same document multiple times by using two forms as "Template"
