Steps To Reproduce:
1)login to module as auditor
2)click on perform audit
3)upload audit record
4)and attach the duplicate documents back to back and observe
5)and also add audit attachments while adding duplicate attachment with same name displays error

Description
Perform Audit > Add audit attachments/upload audit records> uploaded audit attachments are not displayed in perform audit page if user attach duplicate document back to back. But the latest attachment is available in doc pro - assigned level

after applying the patch does not allow to to attach the same document with same name and in perform audit screen
the attachments are over ride and in doc pro assign folder displays blank for the override documnet
