Steps To Reproduce:

1.Login to ewqims
2.Go to problem solver module
3.Problem solver setup→Concern forms
4.Click on new
5.add a element from available element eg : contact number
6.enter form name , click on save
7.Click on view concern evaluation form
8.Click on copy elements from concern creation form
9.click on same element added from available element eg:contact number again click and add element eg:contact number , click on save
10.Observe result

Description

Observation : while adding available elements , two elements are added with same name

Expected result : Only one element should be added , there should be no duplication of elements
