Steps To Reproduce:
1.Login to EwQIMS
2.Navigate to problem solver module
3.Click on Incidents→Actions→Initiate Problem Solving
4.Click on concern title
5.Click on approach link under View forms
6.In Select approach window,Click on Stich document checkbox
7.Select form(s) checkbox to be published
8.Specify order no form(s) need to be stitched
9.Click on Publish
