PDF Template: When clicking on save option the cover page and revision history summary page checkox is checked and the details also got disappeared

Steps To Reproduce:

1. Login to Doc pro module administrator
2. Click on PDF template under the system
3. Check in Header, Footer, Include cover page and Revision history summary
4. Enter the details and save
5. Click on show sample pdf and observe
6. Close the window and again open the same pdf template and observe the Checkboxes and cover page details and then again do the show sample pdf

Description

Issue 1: PDF Template: When clicking on save option the cover page and revision history summary page checkox is checked and the details also got disappeared

Issue 2: When clicking on cover page tab, it is not opening as a new tab.(Always showing under the header and footer details)

Issue 3: When clicks on the revision history summary tab, it is showing under the cover page after saving the template and clicking on Show sample pdf, the cover page details and revision history summary not showing in the show sample pdf

Note: After save the cover page details disappeared from the template screen
