Steps To Reproduce:

1.login into application
2.navigate to change management module
3.initiate PCR change request
4.enter mandatory fields
5.publish the CR form
6.navigate to change request status page
7.open change notification form
8.navigate to document to change sub form
9.enter data's in all section
10.click on submit button
11.login as responsible person
12.navigate to action management
13.observe action description section
