Steps To Reproduce:
1.Login to EwQIMS
2.Navigate to problem solver module
3.Click on Incidents→Actions→Initiate Problem Solving
4.Click on concern title
5.Click on approach link under View forms
6.Click on concern form link(s)
7.Right click on action description,Click on add
8.New row should be added
9.Click on row select <<new>> from dropdown to enter data but cell is automatically
