Steps To Reproduce:

1) Can't able to add Responsible person in D7b
2) Can't able to add permanent corrective actions for D6b
3) If user double click on D4b Description, row is inserting automatically, only after refreshing the form.
4) After filling d4a root cause Occurence, Systemic, Detection. If we refresh the form, D4b is getting added in cell. After getting added, if we deleted the row in d4b, after refreshing d4a root cause getting deleting similarily
