Steps To Reproduce:
1.Login to EwQIMS
2.Navigate to problem solver module
3.Click on Incidents→Actions→Concern Initial evaluation
4.Click on concern title
5.Select priority , teams and other mandatory fields
6.Select approach as 8D/7D/CAR
7.Click on submit
