Steps To Reproduce:

1. Login to CM and Initate a Process change Note
2. Approve the form and initate CN
3. Ctrl +Click on the PTDB link
4. CHeck for the data

Description

Gabriel Customization- The Closed Concern details entered in PS is not populating in the PTDB form.
Concern Closure date, Description, Occurrence and blank.
