On initating CR from FST the form is getting closed automatically and the icon to open the cr form is not clickable

Steps To Reproduce:

1. Login as admin in FST
2. Initiate a CR from VOC page against a requirement
3. On clicking initiate check for the form behavior
4. Click on the icon next to the status for opening CR form
