1)Log in as Mod Admin
2)Click on Documents
3)Click on 'New Doc Request'
4)Select the mandatories field and Template field
5)Click on 'Add' button

Description

1) New Doc Request --> After publishing the document, Template field details are is/are missing in bottom of the screen (Template field Column)
2) New Doc Request --> After publishing the document, should be disabled the template field drop down.
3) New Doc Request --> After publishing the document, should be refresh the template field drop down in on screen.
