Steps To Reproduce:

1.Go to activities and actions page
2.Before. Go to job requirement and select the position and user.
3.CLick detailed competency evaluation and click action training request button
4.Then click edit icon and select date and enter name
5.Click save

Description

Actions page - For action Competency record the data gets duplicated and it shows two times. Refer screenshot

While raise actions the data showing 2 times with different actions name and same description name

Note: The duplicate data is not added in job requirement. Data is not there
