Internal lab Inspection->Reset button is not working & Unable to attach file(Attach icon is not clickable).

Steps To Reproduce:

1) Click on MSA manager link & Enable Internal lab inspection
2)login as a module admin & select the module as MSA pro
3)Click on Group & give Internal lab inspection rights to the user
4)Keep the study is in "out for Repair" status
5)Click on Internal lab inspection menu
6)Click on new & Click on Reset button
7)Click on Attach icon under Inspection Details
