Steps To Reproduce:

1. Login to EwQIMS and navigate to APQP Module
2. Add a Project and add a task to it
3. Attach an Aqua document and set MDR approval
4. Publish the Aqua document
5. Approve draft 1
6. Publish the Aqua document again
=> It gets displayed as a new row in Deliverable Info->Attachments tab
