Steps To Reproduce:

1) Login as admin
2) Click on Failure reason
3) Click on New
4) Select Failure category
5) Now Enter failure reason and save
6) Observe the listing screen

Description

Failure reason is not getting saved even after save it is not listing in Listing screen and also not allows to Edit the Created Failure reason
