Audit form -> We are able assign online form for audit type, but it is deselected after moving to another map entity

Steps To Reproduce:

1) Login as Administrator

2) Click on Module setup

3) Click on Audit Program

4) Enter all the data and save

5) Click on Audit type

6) Click on Audit form

7) Click on + icon and assign online form

8) Click on map entity

9) See the action
