Steps To Reproduce:

1. Login in RFD Module as Module Administrator
2. Click the Main Menu.
3. Click on the PI Tab select the PI
4. Click on the RFD Tab
5. Click on Functions Net
6. Click on Publish for Approval
7. Click on Published Document link
8. Observe the Result
9. Check the Issue in Failure Tree and EFC Tree
